Countries that partake in forms of Euthanasia

Today, forms of passive Euthanasia are legal in many countries, and Active Euthanasia still remains illegal in most.
The only countries that currently legally partake in Active Euthanasia are Mexico, Japan, the Netherlands, and some states in the US. All forms of Euthanasia (even legal) are not considered legal without a patients consent. What that means is that if a family member feels that their 'loved-one' is going through alot of suffering and the 'loved-one' is unable to communicate, the family is not allowed to choose on their behalf. The same scenario goes with a family that have a newborn child suffering.
